About the role:
Reporting to the Manager of Clinical Services you will play a critical role in providing clinical direction and support to the after hours team and our Support Workers with medication, service delivery issues as well as facilitate action required for organisational issues which may arise. You will educate, provide guidance, training to Support Workers and show commitment to best practice and clinical excellence whilst ensuring our clients receive the highest standard of care.

We require flexible availability as you will be employed part time (12 hours per fortnight) and alternating Saturdays and Sundays. The position also requires an on call presence every second week.

Statewide Progressive Neurological Disease Service

Clinical Care Liaison

RN Grade 3/ AHP Grade 2

We are seeking a highly motivated individual with a demonstrated interest in person-centred care and care planning to join our Statewide Progressive Neurological Disease (PND) Service. The Statewide PND Service is a specialist provider of interdisciplinary assessment and management services for people with complex needs due to a diagnosed PND such as Motor Neurone Disease, Huntingtons Disease, Muscular Dystrophy, Multiple Sclerosis and Parkinsonian Syndromes.

The Clinical Care Liaison provides comprehensive care and management advice to clients diagnosed with a PND and their families. This role involves care planning via phone triage, needs analysis, appointment scheduling, shared care planning and liaison with members of the interdisciplinary and research teams. The Clinical Care Liaison also carries a case load and works closely with Neurologists to provide specialist care. This role also involves provision of education and secondary consultation advice to external health care providers.

The successful applicant will demonstrate:
- Highly developed interpersonal skills and professionalism
- The ability to coordinate client care
- The ability to identify and respond to the changing needs of individuals with a PND
- The ability to work collaboratively and effectively within a multidisciplinary team

Applicants must be eligible for on-going entitlement to work in Australia and hold a current and valid Victorian Driver's Licence
